Instructions:

1. Click on form to open.
2. Complete, print, and sign form.
3. Submit completed hiring documents to Financial Aid Office.

• Hiring departments will complete all documents with student.
• U.S. citizens and permanent residents can obtain a Student Employment Referral through their SalukiNet account.
• International Students can obtain a Student Employment Referral at the International Students and Scholars Office.

The following Student Employment hiring documents, in addition to the Student Employment Referral, should be printed and completed by the student at the hiring department once employment has been secured.

• Employment Eligibility Verification (I-9) Form
• Federal/State Withholding Allowance Certificate (W-4)
• Employment Data form
• Authorization for Electronic Direct Deposit (EDD) of Student Pay
• Authorization for Electronic Direct Deposit (EDD) of Student Pay Instructions
• Authorization for Saluki Cash Card (Payroll)
• Ethics Training for Student Employees. Students are expected to read the training document and print and complete the signature page.

U.S. citizens & permanent residents should bring these forms to the Financial Aid Office. International students will take their forms to the Resident Tax Specialist at the NW Annex, Bldg. B, Rm. 127
